How to Assign ICD-10 Diagnosis Codes

 

This requires version Office Therapy version 11.5.0 or higher.

Within Office Therapy, go to your Clients list. Select the client of concern, and then click the Modify button, or you can instead double-click. This will bring up the Client Setup window. Click the Diagnosis tab.

If this is a new client, you will need to Add a new diagnosis. Otherwise, if this is an existing client who has an existing DSM-IV and/or ICD-9 code, select the current diagnosis in this list and then click Modify.

If you are presented with the Diagnosis Date Range screen, and if the client has multiple diagnosis entries, make sure you have selected the current diagnosis and confirm the date range is valid. Click Next.

If you are running Office Therapy version 11.6.0 or higher and in ICD-10 Mode (only on or after October 1, 2015): If there is a Legacy ICD-9 code in the blue box on the right, Office Therapy will automatically try to convert it for you.

Our focus is the list of 12 diagnoses on the left side of the screen. You can hand type either an ICD-9 code or an ICD-10 code. If you enter an ICD-9 code, Office Therapy will look up the corresponding ICD-10 code. If there is a one-to-one match, Office Therapy will automatically insert the ICD-10 code for you. However, if there is more than one available ICD-10 code that can map to the ICD-9 code you have entered, Office Therapy will ask you to select which ICD-10 code and description should be used.

To demonstrate this, you can enter a code such as “300.00” in the ICD-9 column, and then click out of the box or press the Tab key. Office Therapy will automatically enter “F41.9” in the ICD-10 column, as well as the Description. The screenshot below displays the result:

 

 

Now, try entering the ICD-9 code “300.11” and then click out or Tab out, and Office Therapy will then display a list of available ICD-10 codes that match 300.11. You will then need to select which code to use, and click OK to assign the code.

You can also use the Diagnosis Lookup screen, which is accessed by clicking the ellipsis button to the right of the Description field (immediately to the left of the Red X button). You can select to search by ICD-9 code, ICD-10 code, or by Description.

You can select a Diagnosis from this list and click OK to assign it to the client.

Once all the codes are assigned, click the “Finish” button to close the Diagnosis setup screen. You must then click the OK button on the Client Setup screen to save the changes.

 

How to Delete an ICD-10 Code:

You can click the Red X button on the far right to remove the diagnosis when on the diagnosis assignment screen.
