Office Therapy - Insurance name not showing in box 9d or box 32

If information from box 9d or box 32, the field might exceeds the character limit.   Per NUCC, these fields has a character limit.  In our software, we have extended some of these fields by 2 to 3 characters.  It is recommended if the information is not showing in box 9d or box 32, try to reduce the characters in the insurance name.   Please see NUCC Guidelines for more details.
