Office Therapy - Correcting a Charge When There Is a Missing Diagnosis

You cannot submit an encounter or charge when there is no associated diagnosis for the client. In order to add a diagnosis and re-process the charge, this also applies if you have made changes to the client setup. You need to complete the following actions:

 

In this case, you have created a charge for a client without a diagnosis entry. The charge will appear like this.  There will be a warning letting you know that a diagnosis code has not been set for the client.

 


 If you try to submit the charge as a claim though Claims Manager, you will receive a message that a diagnosis is required.




To correct the charge with no diagnosis code: as noted above, go to the Client setup, to the Diagnosis tab.

1.    Go to View and select Clients

2.    Select the client and click Modify at the bottom of the list or double click the Client

3.    Got to Diagnosis tab

4.    Click Add

5.    Add at least one diagnosis code.  Click Finish when completed

6.    Click Ok to exit the Client setup

7.    Open the Client’s Ledger

8.    Open the Charge.  Check to see the diagnosis code is listed.

9.    Click Ok to reprocess the charge to be re-submitted in a claim.
