The new HCFA format is the 02/12.  If you have noticed missing information or having issues with your HCFA, the format of HCFA has not been converted to the new format. 

HCFA 1500 08/05 example:

HCFA 1500 02/12 example:

To convert all insurance companies to the 02/12 format:

Go to File, select Administration, then choose Maintain Electronic Filing Information

Make sure that Electronic Filing Status is set to active

Click on Insurance Companies Filing Electronically tab

Move ALL Companies from the Excluded to the Included list.  Use the  to move all companies or you can select each individual companies and then use  to move into the Included column.

Use the drop down menu next to Convert all included companies to produce and select HCFA 1500 02/12Format.  

Click on Convert Format.  There are no indication that this was formatted but can be verified by going into the Insurance Setup. 

Click on Cancel.  DO NOT CLICK OK AS THIS WILL TURN ON ELECTRONIC FILING.  If this happens, you will follow the steps above and move all companies to the Excluded column.

A message will ask if you would like to save changes, click No

To convert individually insurance to HCFA 02/12 format or to verify conversion:

Go to the Insurance Companies list

Highlight the insurance and double click to open the Insurance Setup

The Paper Filing Format should show HCFA 1500 02/12 Format. 

If it is set to HCFA 1500 08/05 Format, use the drop down and select HCFA 1500 02/12 Format. Click Ok when finished.
